If you're interested in performing, please provide the following information:

1. Performer Name (individual or group)
2. Contact Information of Representative (phone number)
3. Number of performers & required equipment (e.g., microphones, chairs)
4. Introduction text for the MC (or leave blank if left to the MC)
5. Performance details (songs, order, etc.)
6. Desired duration (including setup and breakdown)
7. Preferred performance date and time (the more flexibility, the better!)
8. Audio format for the performance (CD recommended / smartphone permitted)
9. Other stage-related questions or requests

12. Any available social media accounts
