Follow these steps to apply:

Step 1: Complete the Pre-Application Registration Form
Step 2: Submit the following requirements ONSITE in a long brown envelope:

• Accomplished Application Form: Download Here
• Photocopy of Transcript of Records (TOR)
• Two (2) 2×2 ID photos
• Two (2) sealed Recommendation Forms from former professors or immediate supervisors: Recommendation Form
  (Each recommendation must be in a sealed white mailing envelope)
  Step 3: Pay the Processing Fee: PHP 250.00 at the CASH Office
  Step 4: Pay the Testing Fee: PHP 600.00 at the CASH Office
  Step 5: Claim your Test Permit at the MEd Office.
