โจ Episiotomy โ Meaning & Definition:
Episiotomy is a surgical incision made in the perineum, the area between the vaginal opening and the anus, during the second stage of labor to widen the vaginal opening for childbirth.
๐ Types of Episiotomy:
- Midline (Median) Episiotomy
Incision: Straight down towards the anus
Advantages: Less pain, easier repair, reduced blood loss
Disadvantages: Increased risk of extension into the anal sphincter - Mediolateral Episiotomy (Most common in India)
Incision: Diagonal, directed away from the anus
Advantages: Reduced risk of anal sphincter injury
Disadvantages: More pain & blood loss compared to midline - Lateral Episiotomy
Incision: On the lateral side of the vaginal opening
Status: Rarely used due to increased bleeding - J-Shaped Episiotomy
Incision: Curved away from the anus
Status: Rarely practiced
๐ Indications for Episiotomy:
- Rigid or non-stretchable perineum
- Instrumental delivery (forceps/vacuum)
- Fetal distress requiring rapid delivery
- Shoulder dystocia
- Delivering a preterm baby (to minimize head trauma)
- Delivering a large baby (macrosomia)
๐ Anesthesia Used:
- Local infiltration โ 1% lignocaine (most common)
- Pudendal nerve block (if needed)
- Spinal/Epidural (if already in place)
๐งต Suturing of Episiotomy (Post-Placental Delivery):
- Layer 1 โ Vaginal mucosa
- Continuous locking sutures
- Layer 2 โ Perineal muscles
- Interrupted or continuous sutures
- Layer 3 โ Skin (Perineum)
- Subcuticular or interrupted sutures
๐ Suture Material:
- Absorbable: Chromic catgut, Vicryl
๐ฉโโ๏ธ Key Nursing Points:
- Maintain strict aseptic technique
- Monitor for bleeding & hematomas
- Provide perineal care & pain relief
- Educate mothers on hygiene & wound care
๐ก Exam Pearl:
Mediolateral episiotomy is preferred as it significantly reduces the risk of anal sphincter injury.
